How to Get a Medical Cannabis Prescription in Thailand in 2026

Since 23 June 2025, cannabis flower in Thailand has been restricted to medical use under tighter controlled-herb rules. In 2026, additional requirements are changing which types of service locations may receive new licences to dispense cannabis flower.

The compliant route involves:

1. A medical review by an authorized Thai practitioner.
2. A PorThor 33 prescription—officially Form ภ.ท.33 or PT33—when medically appropriate.
3. Dispensing through a licensed location permitted to provide medical cannabis flower and retain the required records.

The three-step PT33 route

2. Complete the medical review

An authorized Thai practitioner reviews your symptoms, history, current medicines and possible risks.

The practitioner—not Cannabox, a dispensary or its staff—decides whether cannabis flower is medically appropriate, how much may be prescribed, how it should be used and the treatment duration.

Approval is never automatic or guaranteed.

3. Use approved documentation correctly

If the practitioner approves treatment, follow the dosage, quantity, duration and other instructions recorded on the PT33 prescription.

Use the prescription only through a licensed service location permitted to dispense cannabis flower and retain the required patient and dispensing records.

A PT33 prescription may cover no more than 30 days of medically necessary treatment per issuance. The practitioner may authorize a shorter period or decline treatment entirely.

What changed in 2026?

Thailand’s 2025 controlled-herb rules established the medical-use requirement for cannabis flower. Online cannabis-flower sales, vending-machine sales and cannabis advertising remain prohibited. DTAM’s 2026 establishment guidance introduces a tighter medical-service model for new licences and renewals. The main public-facing categories are:

• Licensed clinics and healthcare facilities
• Licensed modern-drug pharmacies
• Licensed herbal-product shops
• Legally recognized folk-healer practice locations

This transition means a location that previously operated as a conventional dispensary should not automatically be assumed eligible for future licensing. Patients should confirm that the selected location currently holds permission to dispense cannabis flower and can retain the required PT33 record.

Can tourists and expats apply?

Tourists and foreign residents with a legitimate medical reason may be assessed through a Thai medical pathway. Nationality alone does not determine approval.

A foreign prescription, medical cannabis card or doctor’s letter does not replace the Thai-issued documentation required for cannabis flower in Thailand.

Thailand’s official tourist notice states that visitors must not buy, use, carry or transport cannabis flower without a valid prescription issued in Thailand. If treatment is approved:

• Keep your Thai-issued documentation available.
• Follow the prescribed quantity and treatment duration.
• Use a licensed dispensing location.
• Do not take cannabis across an international border.
• Do not assume that a foreign cannabis card gives legal protection in Thailand.

Who can issue PT33?

PT33 must be issued by an authorized Thai practitioner acting within the treatment guidelines and professional scope applicable to their licence.

The official PT33 form identifies the following categories:

• Medical doctors
• Dentists
• Pharmacists
• Thai traditional medicine practitioners
• Applied Thai traditional medicine practitioners
• Traditional Chinese medicine practitioners
• Authorized folk healers

A dispensary cannot issue PT33 independently or promise that an application will be approved. The clinical decision belongs to the authorized practitioner.

What PT33 does—and does not do

PT33 is Thailand’s prescription record for controlled cannabis flower. It records information including:

• Patient identity
• Practitioner identity and licence details
• Diagnosed disease or condition
• Daily dosage
• Treatment duration
• Total quantity prescribed

PT33 is not:

• A recreational cannabis card
• An unlimited purchase pass
• A guarantee of approval
• Permission to buy cannabis flower online
• Permission to advertise or resell cannabis
• Permission to carry cannabis across an international border
• A guarantee that every location can dispense against the same record

Confirm the dispensing and recordkeeping arrangements before purchasing cannabis flower.

Safety before you apply

Tell the practitioner about:

• Prescription and non-prescription medicines
• Supplements and herbal products
• Sedatives or sleeping medicines
• Alcohol or other substance use
• Allergies or previous adverse reactions
• Pregnancy or breastfeeding
• Heart or cardiovascular conditions
• Any history of anxiety, panic, psychosis, bipolar disorder or depression

Do not drive, operate machinery or perform safety-sensitive work after using cannabis.

Cannabis may cause impairment, anxiety, dizziness, changes in perception or other adverse effects. A medical review does not make cannabis risk-free.

Seek urgent medical assistance if you experience severe confusion, chest pain, loss of consciousness, difficulty breathing, extreme agitation or thoughts of harming yourself or someone else.
